Stage four, a 160 km route starting in Sillian, Tirol, Austria, just a few kilometers from the Italian border, could shake up the standings. After the start, the race immediately returns to Italy to climb to Lake Misurina. A long descent sets up the brutal Passo Sant'Antonio, followed by the more manageable Kreuzbergpass/Passo Monte Croce Comelico.

The route briefly touches Innichen/San Candido before returning to Tirol for a difficult finale. Three climbs come in quick succession: the Anras Oberried KOM, the Tessenberg ascent, and the Kartitscher Sattel KOM - the latter featuring a particularly steep middle section. From its summit, just 7 km remain, mostly downhill to Obertilliach.

The final 3 km follow a wide, gently descending road until the flamme rouge. After the biathlon center, the gradient kicks up again at 4-5% for the uphill finish in Obertilliach's center.
